Responsibilities:
Position Summary:
The Case Manager accomplishes patients’ care by assessing treatment needs developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.

Required Qualifications:
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
- Must demonstrate customer service skills appropriate to the job
- Excellent written and verbal communication skills in English
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
- Required: • All external candidates must have a Bachelor of Science in Nursing • Minimum Education: Current California RN license
- American Heart Association BLS Certification
- Minimum Experience: Three to five years of experience in an acute care hospital setting (medical/surgical preferred)
- Previous one to two years of experience as a Case Manager.
- Must be proficient in typing.
- • Ability to write and communicate professionally
- • Must be proficient in computer skills including Microsoft Office Preferred: Broad knowledge of Medicare, Medi-Cal and insurance guidelines.
- • Critical care experience (For Resource Case Manager)
Preferred Qualifications:
- Knowledge of University and medical center organizations, policies, procedures and forms
