About Resilient Roots Family Wellness Center (RRFWC)
The RRFWC serves as a community space offering:
- Outpatient psychotherapy and medication management for all ages
- Intensive outpatient programs (IOP) for perinatal mental health and children ages 8-12
- A therapeutic preschool with inclusion classes for children 6 weeks to 5 years (pre-K)
- Community events and resources
Ideal candidates are passionate about supporting parents, infants, children, and families in reaching their mental health and educational goals.
Primary Responsibilities
Intake/Discharge Coordinator Duties:
- Support client experience from initial referral to engagement, including answering questions, screening for participation, and connecting to additional services
- Assist clients with intake and referral paperwork for IOP and other services
- Obtain pre-authorizations for IOP services
- Coordinate discharge plans with clinical staff and help schedule follow-up appointments for maternal health and mental health as needed
- Collaborate with Resilient Roots behavioral health, educational staff, and the larger ABHS administrative team
- Complete additional administrative tasks as assigned
- Strong oral and written communication skills that reflect professionalism and knowledge
- Ability to build rapport quickly with children, adolescents, and families
- Effective teamwork and data management skills
- Detail-oriented and responsive to feedback
Salary: $38,000-$44,000
