Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County.

The Recovery Coach will offer a broad spectrum of clinical and psychosocial supports to individuals impacted by Opioid Use Disorder and other substance-use related conditions. Support will include enhanced outreach and engagement techniques, care coordination, resource connection, and emotional/counseling support.
The Recovery Coach will also work closely with patients receiving Medication for Opioid Use Disorder (MOUD) at Marin Community Clinic (MMC) - supporting the patient’s recovery, improved functioning, skill development, and management of chronic health conditions.

The Recovery Coach is a patient-facing role that aims to build engagement and trust with vulnerable patients experiencing clinical and social challenges related to pain, chronic conditions, and opioid use.

Clinic Locations:

San Rafael and Larkspur Clinics.

Responsibilities:

  • Identify and engage individuals who would benefit from Recovery Coaching supports.
  • Provide intensive case management supports to a panel of patients with Opioid Use Disorder and/or other related conditions.
  • Conduct assessments and create tailored care plans with individual patients.
  • Triage and respond to tasks/care needs.
  • Care coordination with MCC’s MOUD team.
  • Ensure that all program reporting requirements are met.
  • Internal and external resource connection/referrals
  • Track referrals and provide follow-up to ensure service delivery.
  • Assist identified patients to coordinate all necessary care.
  • Conduct home/community visits as needed.
  • Consult/coordinate with interdisciplinary care team, including behavioral health, care navigation MAs, nursing, medical providers, and external partners.
  • Attend internal and external meetings (i.e., case conferencing, administrative)
  • Attending trainings such as health coaching, engagement techniques, etc.
  • May travel between clinics or meet Patients in the community as needed.
  • Provide frank and considered opinions regarding case management services and quality improvement measures.
  • Provide services to satellite clinics as needed.
  • Other duties may be assigned.

Qualifications:

Education and Experience:

  • High school diploma or equivalent (GED) required.
  • Certified Addiction Certification (CAC) or equivalent experience
  • Must be highly skilled in motivational interviewing, empathic communication and principles of addiction treatment and recovery.
  • Knowledge of state regulations and legal/ethical standards related to patient rights, and client/patient confidentiality required.
  • 1-2 years’ experience (and passion for) providing support to individuals with MOUD highly preferable.
  • Electronic Medical Records, EPIC experience desired but not required.

Required Skills and Abilities:

  • Must be fluent in English with the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports and business correspondence in English.
  • Bilingual skills - Must be able to communicate with patients and partners in Spanish.
  • Effectively present information and respond to questions from clients, customers, and the public in Spanish.
  • Conduct clinical assessment interviews and case management interventions with patients in Spanish.
  • Has excellent organizational skills.
  • Demonstrates a willingness to be flexible and adaptable in all aspects of this work.
  • Individual must be computer literate and knowledgeable in Microsoft Office software programs, and how to conduct searches on the Internet.
  • Understands the complexities of substance use and recovery.
  • Understands the interplay between chronic conditions and psychosocial issues.
  • Demonstrates cultural humility when working with patients.
  • Demonstrates compassion for and understanding of issues related substance use.
  • Is comfortable working with individuals with complex behavioral health presentations.
  • Has passion for helping individuals navigate a complex system of care.
  • Is a team player and welcomes guidance from multidisciplinary team members.
  • Demonstrates initiative, creativity, and enjoys problem solving.
  • Demonstrates a high level of autonomy and is self-directed.
  • Is comfortable working with community partners including community-based organizations (CBOs), Marin County, medical/BH specialists, and insurance providers.
  • Ascribes to the tenets of person-centered, trauma-informed, housing-first care.
  • Demonstrates compassion and respect for individuals served in all interactions.

  • CPR Training Current
  • Valid CA Driver’s License

Physical Requirements:

  • Prolonged standing, walking, and prolonged periods sitting at desk and working on a computer.
  • Must be able to lift to 15 pounds at times when necessary.
