Legacy Hospice is currently hiring a PRN Compassionate Licensed Practical Nurse (LPN) to provide patient care in and around the Charlottesville Area!
Our Licensed Practical Nurses (LPNs) are responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include, but are not limited to, following the plan of care, providing treatment, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Responsibilities:
- Assesses and evaluates patient's status by following the plan of care, regularly reevaluating patient and family/caregiver needs, and participates in revising the plan of care as necessary based on patient status and needs in conjunction with the RN Case Manager.
- Provides a focused physical assessment while being aware of the physical, emotional and spiritual aspects which may influence health, well-being and quality of life.
- Attends and provides support at the time of death (with physician's order) and per local regulations.
- Maintains regular communication with the attending physician and RN Case Manager regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
- Monitors and maintains documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness via-electronic record so that problems, actions, and goals are accurately and clearly stated and changes are reflected as they occur.
Qualifications:
- Graduate of an Accredited Practical Nurse or Vocational Nurse Program.
- Holds current unencumbered license in the state of practice as an Licensed Practical Nurse.
- Minimum three (3) years' experience as LPN in Community Health, Medical surgical/Acute care setting required.
- Hospice/home health experience preferred but not required.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
