Being a Case Manager at Devereux has it's advantages!

The Case Managerin Malvern, PA, you will provide 10 hours per week of case management services to a small case load of clients in which case manager will act as an advocate and provides problem resolution for clients in obtaining needed services.

Additional 30 hours will consist of providing administrative support to the case management department.

You will be offered:

  • Interaction with all levels of care with ability to work independently and also part of a case management team
  • Flexible Schedule (mostly during the hours of 9am-6pm, with some evening work needed due to family availability)
  • Benefit eligibility after 30 days of employment (1st day of the 1st pay period after 30 days)
  • Pay rate of $19 - $21.66 an hour, dependent upon experience

As a Case Manager you will:

  • Coordinate care for clients with mental health, medical, education, placement and related areas, while maintaining a small caseload (10hrs per week)
  • Coordinate, monitor and reviews the provision of services to ensure clients are receiving and participating in services on a continual basis. Links client to approriate resources
  • Maintains direct contact via telephone and face to face with caseload and parent/guardian per state regulations
  • Ensures timely and accurate completion of related documentation
  • Assist outpatient therapists with administrative responsibilities
  • Assists Blended Case Managers and maintains their resource book
  • Completes ACM tracking and requesting IEP, PE and physicals from providers

Devereux Pennsylvania Children’s Services (DPACS) serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives.

Qualifications:

EDUCATION: BA/BS in a Human Services discipline

EXPERIENCE: Experience is a plus
