Responsibilities:

  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop and update individualized care plans based on assessments and changing patient conditions.
  • Administer medications, treatments, and therapies as prescribed.
  • Monitor patients' vital signs and health status, making necessary adjustments to care plans.
  • Educate patients and their families on disease processes, treatment plans, and self-care techniques.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate care.
  • Communicate changes in patient conditions to the appropriate team members.
  • Maintain accurate and up-to-date patient records and documentation of care provided.
  • Complete necessary paperwork for billing and compliance with healthcare regulations.
  • Provide education and support to patients and families regarding health management and resources available in the community.
  • Advocate for patients’ needs and preferences in care decisions.
  • Adhere to agency policies, procedures, and regulatory requirements to ensure quality patient care.
  • Participate in ongoing training and professional development to maintain licensure and improve skills.

Qualifications:

  • Current RN license in the state of practice.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Previous experience in home health or community health nursing is desirable.
  • Certification in Basic Life Support (BLS) is required.
