Director of Wellness

Overview:

As a member of the management team, the Director of Wellness, RN role will be that of a counselor to residents and families from, or prior to, admission to Hunt Community and to assist residents and their families in all steps of adjustment and transition within the facility.
The Director of Wellness will assist residents and families in dealing with changes due to illness, deterioration as a result of age, dying and death. This individual will be involved in matters relating to staff and resident relationships as well.

The individual must possess a particular understanding of the senior population and the emotional and mental conflicts that arise in a retirement community environment.

Typical schedule Monday-Friday (No on-call)

Responsibilities:

  • Responsible for completing the medical approval assessment for Independent Living waitlist candidates.
  • Visit all new residents, assists with transition, and communicate any needs to the appropriate Department Head.
  • Assists with setting up strategies for maintaining independence.
  • Establishes strong relationships with residents and advocates for services with medical needs.
  • Maintains appropriate records on all resident needs.
  • Provides wellness educational forums and support groups Independent Residents.
  • Works closely with Senior Primary Care to identify plans, assists with choosing appropriate Physicians.
  • Encourages resident independence with health care management as much as possible.
  • Tracks hospitalization and coordinates the readmission of independent living residents to appropriate level of care.
  • Follows up on progress and communicates issues to families and Director of Nursing.
  • Assist residents during temporary or permanent transitions from one level to another.
  • Assists with Independent Living apartment emergency calls if needed as well as assist with sending residents out as needed.
  • Works closely with the Director of Nursing to resolve any care concerns.
  • Coordinates Doctor/Nurse Practitioner schedules for independent living appointments and usage of office space.
  • Organizes resident foot clinic, blood pressure clinic, Podiatrist, flu clinic, and weekly lab services.
  • Works collaboratively with the Director of Nursing when transitioning from Independent Living to Healthcare. .
  • Maintains good communication with all Department Heads.
  • Attends Resident Care Plan meetings for those in transition, as well as Department Head meetings.
  • Supports management with residents and families.
  • Addresses any needs or concerns of Independent Residents.
  • Assumes responsibility for self-safety and that of immediate work environment.
  • Exhibit a high level of compassion in all interactions.
  • Attends annual in-services sponsored by The Hunt at Nashua including, but not limited to, body mechanics, right-to-know information, and fire and safety procedures.

Qualifications:

  • At least an Associate's Degree from an accredited college, preferably a Bachelor's Degree in a Human Service area.
  • Must exhibit evidence of good health and an ability to work effectively with others.
  • Must possess good communication skills and demonstrate willingness to further education in related field.
  • At least five years’ experience in a related area either acute care setting or long-term care.
  • RN license required
  • Social Work experience is a plus.
