Job Summary:

This is a 59-day assignment requiring a Monday to Friday schedule, with possible holiday shifts. The Licensed Master Social Worker will provide vital social work services within a hospital setting, focusing on case management and discharge planning.

Responsibilities:

• Conduct comprehensive assessments to determine patients' social and emotional needs.
• Develop and implement effective discharge plans, collaborating with the interdisciplinary care team.
• Manage a caseload efficiently, demonstrating robust multitasking abilities.
• Utilize EPIC EMR for patient management and documentation.
• Handle Allscripts for referral management and coordinate transportation setups as needed.
• Interact and collaborate with various healthcare professionals to ensure comprehensive patient care.
• Support psychiatric and pediatric populations, addressing their unique needs.

Qualifications:

• Master’s Degree in Social Work (MSW) (required).
• Licensed Master Social Worker (LMSW) by the State of New York (required).
• Previous hospital social work experience (required).
• Case management experience with a focus on discharge planning (preferred).
• Strong computer navigation skills and familiarity with EPIC EMR and Allscripts (preferred).
