RN Case Manager

Compensation: $85 to $90 per hour

Duties and Responsibilities -

Conduct thorough assessments of patients' medical, physical, and psychosocial needs.-

Develop and implement individualized care plans, considering patient goals and available resources.-

Collaborate with physicians, nurses, and other healthcare providers to ensure comprehensive care.-

Coordinate patient care services across various settings such as hospitals, clinics, home care, or long-term care facilities.-

Ensure smooth transitions of care between healthcare providers or settings (e.g., discharge from the hospital to home or rehabilitation). -

Act as a liaison between patients, families, and healthcare providers.-

Review patient cases to ensure appropriate use of healthcare services, avoiding unnecessary treatments or hospital readmissions.-

Submit reports and documentation to insurance companies for authorization and reimbursement.
