Responsibilities:
Delivers quality primary care and/or specialty care services to clients/patients who are HIV-positive, Hepatitis C infected, seeking PrEP/PEP and/or others, including behavioral health patients accessing primary care services as required by the work of the Department.Maintains and exhibits consistently positive attitudes and functions in a solution-oriented manner to facilitate the work of HOPE Center. Supports Medical Director’s administrative responsibilities through work on policies and procedures, quality improvement activities, and implementation of current DOH and HRSA programs to reduce the number of people living with HIV/Hep C in NY State.
Qualifications:
Master’s level Family or Adult Nurse Practitioner graduate from an accredited school. DNP preferred. Qualified by training and experience for certification or has received certification in a specialty which includes primary health care services, and is qualified by experience and demonstrated competency as determined by the governing authority and has an active DEA number.Experience demonstrating the ability to plan, organize, develop and implement goals, objectives, policies and procedures necessary for quality care. Experience embracing and adapting to change required. Computer operations required. Identifies diagnoses and treats actual and potential health care problems for individuals across the life span in collaboration with a family practice physician (or equivalent).
Such services are performed in accordance with a written practice agreement and written practice protocols. HIV Specialist, as defined by the New York State Department of Health is preferred. Other HIV specialist designations are preferred. Experience treating individuals with Hepatitis C is preferred.
Knowledge of database design and database management preferred. Bilingual (Spanish/English) preferred.
