Responsibilities:

The Central Community Health Partnership is a collaboration of premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health Community Partner and Long-term Services and Supports through the MassHealth program.

Finding the right combination of services and knowing how to access them can seem to be an overwhelming task for those with Behavioral Health Challenges and Central CHP stands ready to help navigate the array of choices. The vision of Central Community Health Partnership is integrated care that ensures that every member receives the right care at the right time – making good health and wellness a reality, not a dream.

The BH Nursing Facility Care Coordinator, in collaboration with the Care Manager and Registered Nurse, will serve as primary point of contact for Nursing Facilities across our service area by providing outreach, engagement, assessment and person-centered treatment planning for individuals enrolled in the Central Community Health Partnership’s Behavioral Health (CCHP/BH) Community Partners Program.

The BH Nursing Facility Coordinator will act as a liaison for all Skilled Nursing Facilities and build relationships that support enhanced coordination for individuals served.

Other Key Responsibilities:

  • Develop and maintain relationships with all Skilled Nursing Facilities to ensure collaboration for individuals served.
  • Develop and maintain relationship with MassHealth and Department of Mental Health. Attend meetings with MassHealth and Department of Mental Health as needed.
  • Track all referrals made on behalf of members residing in skilled nursing facility. This includes referrals made by CP along with referrals made by Skilled Nursing Facility and other providers. Ensure information is captured in quarterly report to MassHealth.
  • As individuals in Skilled Nursing Facilities become appropriate to transition back into the community, support the Skilled Nursing Facility to assist individuals in this transition.

Qualifications:

  • High School Diploma/GED and/or applicable experience in community-based behavioral health support programs as a community health worker, peer specialist, or recovery coach may be substituted for degree.
  • Valid Drivers License, reliable transportation and acceptable driving record, required.
  • Community Health Worker (CHW) Certification Required. If candidate does not have an active CHW certification, must be willing and able to obtain within the first 6 months of employment.
  • Must be knowledgeable in Skilled Nursing Facilities, dementia, and different layers of guardianship.
  • Experience working with adults with developmental disabilities/geriatric population, experience with accessing local resources, navigating health, behavioral health, and/or substance abuse treatment systems.
  • Must have excellent communication, documentation, time management, and organizational skills.
