Duties and Responsibilities: Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources)Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
Liaison between family and other interdisciplinary team members

Qualifications & Requirements: LMSW preferred
