Job Description:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our Long-Term Care unit in Montour Falls, NY. The ideal candidate will provide direct nursing care to residents, ensuring their safety, comfort, and well-being while adhering to facility protocols and state regulations.

Key Responsibilities:

• Provide direct patient care, including monitoring vital signs, administering medications, and assisting with daily activities.
• Administer prescribed medications and treatments in accordance with facility policies and state regulations.
• Assess, clean, and dress wounds to promote healing and prevent infections.
• Maintain accurate medical records, including progress notes, assessments, and care plans.
• Assist Registered Nurses (RNs) and physicians in preparing residents for exams, procedures, and treatments.
• Monitor residents’ conditions, report health status changes to RNs or physicians, and participate in care planning.
• Follow infection prevention protocols, promote hygiene, and enforce safety measures.
• Communicate with residents and their families regarding health conditions, medications, and care plans.
• Supervise Certified Nursing Assistants (CNAs), ensuring they provide appropriate care and assisting them as needed.
• Respond quickly in medical emergencies, providing first aid, CPR, and escalating concerns to medical staff.

Requirements:

• 2+ years of recent experience as an LPN/LVN in a Long-Term Care unit.
• Active New York State (NYS) LPN License.
• AHA BLS Certification.
