This position requires a wound care certified nurse practitioner who is experienced, team-oriented, and able to provide expert wound care in the home setting using an application-based electronic patient record, according to the practice guidelines of the state where the service is provided.The nurse practitioner will provide and oversee wound care, diagnose, and treat wound care patients following evidence-based guidelines, monitor clinical quality of care, and ensure regulatory and company compliance within the assigned region.
Qualified candidates will have strong knowledge of evidenced-based wound care practices and strategies to improve targeted outcomes. The wound certified nurse practitioner will work closely with the assigned team to monitor outcomes, clinical efficiency, appropriateness/quality of care and supervise care.
Responsibilities:
- Demonstrates proficiency in performing the full scope of wound care and management, including patient health interviews, review of medical/surgical health history, physical assessment, ordering and interpretation of diagnostic testing, diagnosing health problems, managing health care, providing health teaching and counseling, initiation of referrals, following Wound Healing Guidelines (WHG) for indicated treatment and follow-up scheduling, collaborating with assigned team members and other health care providers, and maintaining electronic health records.
- Prescribes appropriate treatment orders in accordance with the WHG for wound dressings, laboratory testing, cultures, imaging, specialty/offloading, and pressure reducing devices.
- The advanced practice nurse may prescribe non-scheduled legend drugs, initiate, monitor, alter or order drug therapies based on educational preparation, clinical experience, and state guidelines/scope of practice either in collaboration with a physician provider or under the supervision of a physician provider as required by the state law where the care is delivered.
- Implement and maintain professional patient communications during in-home visits and via the application-based electronic patient record.
- Complete documentation that meets regulatory guidelines and ensures compliance with documentation in the application-based electronic patient record for clinical care instructions, communications, wound care supply orders, and diagnostic testing results.
- Maintains and ensures regional compliance with all regulatory and company standards.
- Completes and monitors appropriate notices for discharge including referral to the case manager for post-healing follow-up within the assigned region when indicated.
- Demonstrates safe work practices including proper body mechanics, proper infection control precautions, and takes corrective action when safety risks are encountered.
- Complies with company policies and procedures.
- Assesses evidence of abuse and demonstrates knowledge of abuse reporting.
- Provides ongoing patient/caregiver education.
- Performs procedures, dependent on the patient’s condition and provider aptitude as accepted by the Board of Nursing in the state where the care is delivered. Procedures include incision and drainage, excisional debridement, non-excisional debridement, hematoma evacuation, suture removal, puncture or needle aspiration, application of all types of wound dressings including compression wraps, negative pressure wound therapy, and bio-engineered skin substitutes.
- Makes practice decisions ethically with consideration of safety, effectiveness, and cost in planning and care delivery.
- Initiates effective action in emergencies following company policies and procedures.
- Maintains standard and transmission-based infection control precautions appropriate for each patient.
- Remains available during scheduled work hours for clinical guidance and appropriate patient care activities.
- Works collaboratively with all departments in the company.
- Available to oversee RNs and LPNs in the assigned region and provide orders and patient care guidance as needed.
- Available as a resource for assigned regional staff.
- Participates in product and treatment evaluations.
- Demonstrates strong customer service skills. Addresses customer complaints within the region in conjunction with the compliance department.
- Maintains knowledge of HIPAA Privacy, Security and Breach Notification Rules.
- Current, unencumbered nursing license in the state of practice.
- Reliable internet access, cell phone supplied by company.
- Willingness to participate in a call schedule.
- Reliable transportation and an unrestricted driver’s license.
Qualifications/Education:
- Certified through ANCC (American Nurses Credentialing Center) or AANP as a Nurse Practitioner
- WOCN (Wound, Ostomy and Continence Nurses) certification or other company approved wound care certification required
- U.S. Citizenship
- Currently licensed as a professional nurse and advanced practice nurse in good standing
- Proficient in English
- Meets physical standards
- BLS certification
Experience:
- Minimum of 3 years’ experience treating chronic wounds in a hospital, outpatient, or post-acute setting
- Direct patient care experience
- Electronic patient records proficiency
