Clinical Specialist Pharm - Miami
Overview:
The Clinical Specialist Pharmacy works in collaboration with other members of the patient care team to manage patientcare outcomes. This position provides drug information services and assists in solving patient condition- and drugrelated problems.Assists with delivery of service regarding drug monitoring and drug regimen adjustments while working both directly and indirectly with the patient.
Responsibilities:
- Provides medication and disease state education to patients, both face-to-face during the patient’s hospitalization and/or via telephone communication post-discharge.
- Performs comprehensive medication therapy reviews for patients targeted by the MTM program. Assesses for medication-related problems and introduces therapeutic solutions.
- Collaborates with prescribers to maintain accurate post discharge medication reconciliation and resolve medication related problems.
- Collaborates with nurse care coordinators to identify and resolve medication related problems, non-adherence, medication access problems, and insufficient patient knowledge after discharge from the hospital.
- Monitors and reports to admitting floor pharmacist special considerations of boarded patients. Provides Medication
- Collects and analyze data pertaining to medication-related problems and the overall performance of the MTM program.
- Assists in planning for ongoing optimization of the MTM program, including but not limited to: expansion utilizing APPE pharmacy students, development of a residency program, development of disease management protocols and collaborative practice agreements, facilitate reimbursement for MTM services, etc.
- Participates on committees regarding medication management.
Qualifications:
Education / Training- Graduate Pharmacy Program
Preferences:
Doctor of Pharmacy Degree. PGY1 residency and PGY2 residency in ambulatory care, geriatrics, managed care, or transitions of care preferred. Board certification by the Board of Pharmacy Specialties (BPS) in either pharmacotherapy or ambulatory care; or Certification in Geriatric Pharmacy (CGP) preferred.
Experience Requirements- 2-years Clinical Pharmacist Experience
Preferences:
2 years experience with either Critical Care, Cardiovascular, OB/NICU/Pediatric, or MTM experience preferred.
Certificates/Licenses/Registration- Licensed Pharmacist in Florida
- Basic Life Support (BLS)
- Advanced Cardiovascular Life Support (ACLS)
Preferences:
Board certification by the Board of Pharmacy Specialties (BPS) in either pharmacotherapy or ambulatory care;or Certification in Geriatric Pharmacy (CGP).
Additional Information:
