Job Summary:
The Community Health Worker (CHW) serves as a vital link between the community and healthcare providers. This role involves promoting health education, facilitating access to healthcare services, and supporting individuals in managing their health and wellness.The CHW will work closely with diverse populations to address health disparities, improve health outcomes, and enhance the overall well-being of the community.

- Health Education and Promotion:
- Provide information on health topics, preventive care, and available services.
- Conduct workshops, seminars, and outreach activities to promote healthy behaviors and lifestyles.
- Develop educational materials tailored to the community’s needs.
- Community Outreach:
- Identify and engage with community members to assess their health needs.
- Build relationships with local organizations, schools, and businesses to promote health initiatives.
- Organize and participate in community events to raise awareness about health issues.
- Access to Services:
- Assist individuals in navigating the healthcare system, including scheduling appointments and understanding insurance options.
- Connect clients with resources such as food assistance, housing support, and mental health services.
- Advocate for clients to ensure they receive appropriate care and support.
- Support and Counseling:
- Provide emotional support and encouragement to individuals facing health challenges.
- Assist clients in setting and achieving personal health goals.
- Facilitate support groups and peer mentoring programs.
- Data Collection and Reporting:
- Collect and maintain data on community health trends and program effectiveness.
- Prepare reports and presentations for stakeholders and funding agencies.
- Participate in program evaluation and improvement efforts.
- Collaboration:
- Work collaboratively with healthcare providers, social workers, and other community organizations.
- Participate in interdisciplinary team meetings to coordinate care and services.
- Share insights and feedback from the community to inform program development.
Qualifications:
- High school diploma or equivalent; associate’s or bachelor’s degree in public health, social work, or a related field preferred.
- Certification as a Community Health Worker (if applicable).
- Experience in community outreach, health education, or social services.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of local health resources and services.
- Cultural competence and sensitivity to diverse populations.
Working Conditions:
- May require travel within the community.
- Flexible hours, including evenings and weekends, may be necessary to accommodate community events and client needs.
- Work may be performed in various settings, including community centers, schools, and clients’ homes.
