For one of our clients, we are looking for a Physical Therapist Case Manager: Home Health, remotely from California, United States.
As a Physical Therapist Case Manager, you will play a vital role in providing skilled therapy services to patients in their homes, focusing on mobility, rehabilitation, and independence. You will assess patients' conditions, develop personalized treatment plans, and work alongside an interdisciplinary team to deliver high-quality care.This role requires strong clinical judgment, excellent communication skills, and a compassionate approach to patient care. Additionally, you may supervise Physical Therapy Assistants (PTAs) in accordance with agency and regulatory standards. Your primary goal will be to help patients regain functionality and improve their overall quality of life within the comfort of their homes.
Accountabilities:
- Conduct home-based physical therapy evaluations and develop individualized care plans.
- Implement treatment programs to help patients restore mobility and prevent disability.
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care.
- Supervise and guide PTAs to ensure adherence to treatment plans and best practices.
- Monitor patient progress, adjust care plans as needed, and provide timely documentation.
- Educate patients and their families on treatment plans, exercises, and lifestyle adjustments.
- Active and unrestricted California Physical Therapist License.
- Current BLS/CPR certification.
- Valid California Driver's License.
- Strong understanding of home health regulations and local community resources.
- Prior experience in home healthcare is highly preferred.
- Experience working with high-acuity patients and interdisciplinary teams is a plus.
- Ability to demonstrate empathy, leadership, organization, and critical thinking skills.
- Flexible work schedules and autonomy in managing caseloads.
- Targeted territories to minimize travel time.
