Clinician - Adult Services (DDS/ABI)
Salary: USD $90,000.00/Yr.
Description and Responsibilities:
Open Sky’s Acquired Brain Injury programs serve adults with acquired brain injury and physical disabilities who are referred by the Massachusetts Rehabilitation Commission with the goal of providing individualized services to help adults live independently, in their own apartment.Services include finding accessible housing, coordinating healthcare, supervising PCAs and arranging for transportation.

In this role, the Clinician will provide mobile clinical case management support to individuals in Northern Worcester County with Acquired Brain Injury. They will coordinate with other members of the treatment team including family members, DDS, day programs, personal providers, and allied health providers including the VNA.
Other Key Responsibilities:
- Establish engagement with individuals served employing Motivational Interviewing/Cognitive Behavioral Therapy skills.
- Participate in treatment planning in order to develop appropriate, effective and achievable treatment goals and objectives.
- Facilitate therapeutic groups with people served/staff.
- Prepare Individualized Positive Behavioral Supports, Targeted/Intensive Plans and Medication Treatment plans for individuals living in the homes as clinically indicated.
Qualifications:
Master's Degree in Social Services field, experience with assessment and treatment planning for adults with ID, ASD, and/or brain injury/ neurological issues, and a valid Driver’s License and acceptable driving record, required.
