Who we're looking for:

Under direction of the licensed nurse, this individual will be responsible for assisting with the initial nursing intake on new admissions, gathering demographic data and assuring that referral documents are present and available. This individual will assist in scheduling appointments for clients with outside providers, as well as completing internal paperwork for these appointments, such as trip logs, referral forms, and follow up appointments.
This individual will be responsible for routine monitoring, such as vital signs and weights, and recording this information in the approved format. In addition to duties as described, this individual will be able to monitor the self-administration of medications, as directed by the licensed nurse, record this activity, and communicate all deviation from standard directions.

In assisting nursing staff, this individual will conduct regular inventory of supplies, maintain logs, and communicate any observations and interactions of a clinical nature to the licensed nurse on duty. Under direction, this person will also perform regular audits of medical records, reviewing for compliance with regulations.

Work is performed under the supervision and direction of licensed nursing personnel in accordance with established regulations, policies and procedures of the agency. Work is also performed in accordance with the Nurse Practice Act and according to established regulations, policies and procedures of the agency.

Work is not performed independently but must be coordinated and assigned by supervisory staff (licensed nursing personnel).

Responsibilities:

Your responsibilities in this role may include:

  • Treat all clients and staff with dignity and respect
  • Conduct routine vital signs and weight assessments on clients and document according to protocol
  • Under direction of a licensed nurse, conduct initial intake interviews on new admissions, collecting information such as demographic data, reported medical and mental health information, and referring documents and/or medication
  • Perform tasks delegated under the supervision of licensed nursing staff to assist with triage of client health complaints/requests
  • Communicate all information obtained to licensed nursing personnel
  • Maintain medical/health related forms and upload in the EHR
  • Audit medical records monthly
  • Maintain adequate inventory of medical supplies and accurate records of same in compliance with governing regulations and agency policy
  • Arrange appropriate appointments with Health Care Providers to secure complete physical examinations, psychiatric appointments, required health screenings, and supplemental tests
  • Assist with coordination of referrals for crisis, emergency care, or urgent care as warranted
  • Administer medications following regulations outlined by the nurse practice act and organization policies and procedures, supervise the self-administered of medications where appropriate and allowed by regulation
  • Ensure proper safety and security of medication handling, storage, inventory, control, and administration as well as the proper storage of medical supplies as required by regulation
  • Call in medication and/ or refills as directed by physician
  • Collaborate with licensed nurses for destruction of medication
  • Ensure that clients have appropriate medical coverage to guarantee safety, adequate care, and provider compliance for payment
  • Participate in treatment/multidisciplinary team meetings, as directed
  • Assure adherence to infection control policies and practices
  • Communicate to nurses any identified needs for clients regarding health, nutrition, safety, and medication compliance
  • Maintain CMT license with good standing
  • Maintain CPR/First Aid certification, and provide when needed
  • Required to work every other weekend, both Saturday and Sunday
  • May be required to work shifts outside of assigned shift during staffing shortages
  • Minimum holiday requirement is 1 major summer holiday (Memorial Day, July 4th or Labor Day) and 1 major winter Holiday (Thanksgiving, Christmas, or New Year’s Day (night shift: New Year’s Eve)
  • This position is deemed essential and will be required to work during inclement weather

Education: Active CMT certification required.

Preferred Experience: Minimum of two (2) years supervised experience in a health care setting, working as a Certified Medication Technician
