Responsibilities:
This position will be within Memorial North Park Hospital and Memorial Main Hospital
Collaborate with physicians, hospitals, and care facilities to provide coordinated and quality home-based services. Be the beacon of support and guidance in our healthcare landscape.
Assist discharge coordinators in coordinating home care services, empowering patients to make informed choices for their wellbeing.
Provide patients and families with compassionate education on home care regulations and services.

Qualifications:
RN licensed to practice in the state serviced
Two years of appropriate nursing Experience
