Job Title: Patient Care Coordinator II

Duties:

• Partner with Primary Care Providers to implement and monitor patients' care plans.
• Coordinate appointments, follow-ups, and pre-certifications with insurance carriers.
• Manage reminders, alerts, and dashboard tasks to ensure smooth patient care progression.
• Resolve billing errors and escalate issues to the Department Manager as needed.
• Serve as a liaison between patients, physicians, and clinical staff, connecting patients with internal and external resources.
• Provide thoughtful guidance to patients regarding healthcare options.
• Contribute to process improvements aimed at enhancing patient satisfaction.

Skills Required:

• 4+ years of experience in healthcare, customer service, or related fields.
• Strong communication and organizational skills.

Education:

• Associate’s degree required; Bachelor’s degree preferred.
• Knowledge of medical terminology highly desirable.

Pay Range:

• Weekly: $1,300 - $1,600
