Responsibilities:
The Wound Care Coordinator plays a pivotal role in coordinating wound care services for patients, including identifying home health companies and facilities that accept patients’ insurance, arranging lymphedema therapy treatments, tracking the delivery, and set up of lymphedema pumps, managing physician orders, and ensuring efficient oversight of wound care schedules.

This role will also act as a Patient Services Coordinator (Vein Clinic), as needed, to help patients schedule appointments and give pre and post appointment instructions.

Wound Care Coordinator
  • Identify and research home health companies and facilities offering lymphedema therapy treatments within designated regions that accept patients' insurance for comprehensive care.
  • Establish and maintain effective relationships with home health providers, facilities, and healthcare professionals to facilitate smooth coordination of patient care.
  • Collaborate with physicians and other healthcare providers to obtain necessary patient documentation and orders for wound care and lymphedema therapy services.
  • Verify insurance coverage and eligibility for services, ensuring alignment with the requirements of home health companies and facilities.
  • Coordinate the transfer of patient information, including medical records and orders, to approved facilities and home health companies in compliance with HIPAA regulations.
  • Monitor and track the delivery and setup of lymphedema pumps, ensuring patients receive necessary equipment and support.
  • Document in patient charts the status of home health orders and lymphedema orders, including confirmation of nurse visits for pump setup and ongoing care.
  • Receive and process orders from home health companies requiring physician signatures, ensuring timely completion and submission.
  • Address any questions or concerns regarding patient care from healthcare providers, patients, or caregivers, providing timely and accurate information.
  • Verify oversight of graft management for patients receiving wound care services, ensuring compliance with treatment protocols and guidelines.
  • Oversee the wound care schedule to ensure efficient flow of patients and timely delivery of services.
  • Schedule representative meetings for wound care product suppliers, coordinating demonstrations and product evaluations as needed.
  • Order and maintain inventory of wound care supplies and samples, ensuring availability for patient care and treatment.
Patient Services Coordinator
  • Greet patients upon arrival.
  • Check all patients in and out of their appointments.
  • Administer, explain, and collect new patient paperwork.
  • Scan, save, and enter patient information into electronic health records system.
  • Answer inbound calls within UVVC.
  • Schedule patients’ next appointments to include full treatment plan.
  • Work directly (face to face) with patients to identify appointment times that work, while following the physician’s prescribed orders.
  • Respond to patient questions and give accurate and trustworthy responses.
  • Answer inbound calls from existing patients who need assistance arriving to the clinic.
  • Enter any maintenance issues in facility management software.
  • Make outbound calls to existing patients to support and improve their overall experience.
  • Submit and maintain accurate records in multiple systems such as TYSYS Maintenance Care, Care Credit and the EMR.
  • Participate in team meetings where new processes, policies, and services are discussed.
  • Interact with other internal departments to ensure the highest standard of patient care.
  • Work to retain and prevent cancellations and no shows.
  • Keep accurate records of all cancellations, no shows, and potential patients who do not book an appointment.
  • Open and close the office including locking doors and unlocking doors.
  • Update all records in the EMR and CRM, including PCP, pharmacy, demographic information, and encounters.
  • Upload documents in the patients’ charts.
  • Take patient payments and post to TYSYS and e CW.
  • Ensure the lobby area is neat, organized, and clean.
  • Process all mail received at clinic including checks.
  • Submit, review, and investigate authorizations and referrals for patient upcoming appointments.
  • Update patient charts.

Qualifications:

  • High School diploma or equivalent Some college preferred.
  • Prior experience in wound care coordination, healthcare management, or case management.
  • Comprehensive understanding of wound care processes, lymphedema therapy treatments, home health operations, and healthcare coordination.
  • 1-2 years patient facing, customer service, or sales environment experience preferred.
  • Strong organizational skills with the ability to manage multiple tasks and prioritize effectively and strong attention to detail.
  • Excellent communication and interpersonal skills, with the ability to collaborate with diverse stakeholders.
  • Strong working knowledge in electronic medical record management (EMR) systems and Microsoft Office Suite.
  • Knowledge of insurance verification and authorization processes.
  • Familiarity with HIPAA regulations and patient privacy laws.
  • Able to embrace and adapt to new system technologies to support the business.
  • Dependable; able to meet reliable attendance and punctuality standards.
