The primary care physician empowers patients to take an active role in health maintenance and promote wellness behaviors. Provides appropriate patient follow-up and communicate effectively with patients. Refers patients to specialty care, as necessary.Provides clinical supervision to mid-level providers and nursing staff. Participates in quality assurance activities. Participates in State and National disease collaborative efforts.
