Physical Therapist - Home Health

This role is pivotal in creating a positive and nurturing rehabilitative environment for individuals recovering from illness, injury, or surgery.

Key Responsibilities:

• Patient Evaluation: Conduct comprehensive physical assessments and review medical histories to understand each patient’s condition.
• Customized Care Plans: Develop and implement personalized treatment plans tailored to meet each patient's unique needs and rehabilitation goals.
• Therapeutic Interventions: Employ therapeutic exercises and training programs designed to enhance mobility, strength, and overall functionality.
• Education and Support: Provide patients and their families with essential education on injury prevention, wellness, and steps to recovery, fostering a supportive environment for healing.
• Progress Monitoring: Track and document patients' progress, adjusting treatment plans as necessary to ensure optimal outcomes.
• Collaborative Care: Work closely with other healthcare providers to deliver holistic and comprehensive care.
• Compliance and Documentation: Maintain accurate and up-to-date patient records in line with regulatory standards.

• A Master’s degree in Physical Therapy.
• A minimum of 1 year of experience in home health as a Physical Therapist.
• Active licensure to practice as a Physical Therapist in the state of operation, adhering to professional standards and practices.
• Valid CPR certification.
• A licensed driver with the ability to travel to various locations as required.
