Overview:

Part-time (32 hours per week, benefits eligible)

The Flagler Home Care Physical Therapy Assistant provides skilled therapy intervention and treatment to patients in the home setting, under the supervision of the Physical Therapist. This care includes providing direct patient care according to the treatment plan established by the Physical Therapist.

The Physical Therapy Assistant is responsible for educating patients and caregivers while preparing the patient/caregiver for discharge as appropriate.

Responsibilities:

  • Contributes to the evaluation process by implementing delegated assessments and by providing verbal and written
reports of observations and client capacities to the supervising therapist. Provides input for intervention plan, based on
client needs and priorities and is knowledgeable about the client’s therapy goals.
  • Observe, record, and report the client’s reaction to treatment and any changes in the client’s condition to the supervising
Physical Therapist, physician, Director of Nursing, Administrator and/or Clinical Supervisor as needed and in accordance
with agency policies.
  • Educate and document in the patient record instructions given to patient, caregiver, and/or family member or other healthcare personnel.
  • Submit accurate documentation in accordance with agency policies.
  • Participate in meetings and on committees, as well as educational opportunities to continually improve personal
performance and overall home care excellence. Participate in the agency’s quality assessment and performance
improvement program.
  • Under the guidance of the supervising therapist, treats to relieve pain, develop and restore function and maintain
maximum performance using appropriate treatments, modalities, and procedure as stated in the plan of care.
  • Promote coordination of care through interdisciplinary communication in accordance with agency policies.
  • Participate in instructing the client, family, caregiver and/or other healthcare team personnel in the exercise program
developed by the Occupational Therapist for strengthening and controlling the client’s upper extremities.
  • Uphold the highest ethical standards in relation to interaction and work with patients, physicians, and other community
contacts.
  • Meet the productivity standards that have been established by the agency.

Qualifications:

Education
  • Graduate of an APTA accredited Physical Therapy program required.
Experience Requirements
  • 2 or more years experience in home health preferred but not required.
Certificates/Licenses/Registration
  • Physical Therapist Assistant License in Florida
  • Basic Life Support (BLS)
Additional Information: Level II background check required for position. Must have a good driving record. Valid driver's
license and proof of auto insurance required for Motor Vehicles Record (MVR) background check.

Post-Employment: BLS, provided by American Heart Association (AHA), required within 30 days of hire.
