Pathways Coordinator

SUMMARY: Coordinates and oversees the day-to-day operations of the Pathways Program. Supervises Pathways Job Coaches, including overseeing implementation of and support for all department and agency trainings.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  1. Forges and maintains relationships with a variety of local employment partners to ensure training opportunities for our Pathways Members.
  2. Manages scheduling of Job Coaches and Members for Pathways programs.
  3. Schedules agency vehicles in support of Pathways program.
  4. Trains and supports ES staff and Members (as needed) with any computer or training related matters.
  5. Completes Pathways monthly billing timely, correctly, and accurately.
  6. When completing monthly billing, ensures that progress notes have been completed for hours billed.
  7. Follow-up with Job Coaches to obtain missing progress notes.
  8. Tracks the effective date of authorizations for services. Requests updated authorizations in sufficient time to avoid expiration. Follows-up with Regional Center, as required.
  9. Assembles case record for newly employed Member.
  10. Maintains case records up to CARF and funding source standards.
  11. Attends all Member IPP meetings.
  12. Maintains Member Database so that it is up-to-date and accurate.
  13. Acts as a substitute Job Coach as needed.
  14. Completes all paperwork, documentation, or progress notes timely and correctly.
  15. Responsible for completing Member payroll in the Pathways program
  16. Schedules and monitors Pathways Job Coach hours

OTHER DUTIES AND RESPONSIBILITIES include the following:

  1. Assists with case record reviews.
  2. As needed, substitutes for absent Job Coaches and provides coaching to Members.
  3. Attends all scheduled meetings and trainings.
  4. Support the Agency’s mission and partners with others to accomplish the mission.
  5. Maintains the highest professional and ethical standards at all times.
  6. Performs other duties as assigned.

SUPERVISORY RESPNSIBILITIES: Directly supervises the Pathways Job Coaches. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing and resolving problems.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Knowledge of the needs of persons with developmental disabilities and the ability to relate effectively with persons with disabilities and employers.

The individual must have a reliable vehicle, proof of insurance, a valid California Driver License, and a driving record acceptable to the agency’s insurance carrier.

EDUCATION and/or EXPERIENCE: College level course work in rehabilitation, human service or related field is preferred. Direct experience working in a supported employment program is required. At least two years direct experience working with adults with developmental disabilities is required.

LANGUAGE SKILLS: Ability to read and interpret documents in English. Ability to write reports and correspondence in English. Ability to effectively present information and respond to questions in English from Members, staff, and employers.

The employee is occasionally required to sit and climb or balance. The employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds, and occasionally lift and/or move up to 50 pounds.
