Job Description:

The Licensed Practical Nurse (LPN) will provide patient-centered care in a Long Term Care (LTC) / Nursing Home setting. The role involves working closely with residents, their families, and the healthcare team to ensure the highest standard of care.

Key Responsibilities:

• Administer medications and treatments per physician orders.
• Monitor residents’ conditions and report any changes to the RN or physician.
• Provide direct patient care, including wound care, catheter management, and ADL support.
• Document all care provided in electronic medical records (EMR).
• Work collaboratively with CNAs and other healthcare team members.
• Ensure compliance with facility policies, infection control, and safety protocols.
• Offer emotional support and advocacy for residents.

Qualifications & Requirements:

• Active LPN license (Minnesota or compact state).
• Minimum 1 year of experience in Long Term Care or Skilled Nursing.
• Current BLS certification (American Heart Association preferred).
• Strong clinical skills, attention to detail, and adaptability to a variable schedule.
