Permanent - Primary Care Physician - Geriatrics - Onsite Stockto
We invite applications for a permanent Primary Care Physician in Geriatrics. This role provides specialized care to the elderly, emphasizing individualized treatment and preventive measures in a collaborative, patient-centered environment.
The Primary Care Physician performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant.
At the direction of the Medical Director, the Primary Care Physician oversees all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination.
The Primary Care Physician also provides direction and support to Nurse Practitioners and other members of the IDT.
Responsibilities
Provide comprehensive, patient-centered primary care for geriatric populations, ensuring regular monitoring and management of chronic conditions.
Conduct thorough assessments and formulate individualized care plans tailored to the unique needs of older adults.
Collaborate with multidisciplinary teams to coordinate care, optimize treatment outcomes, and support patients' overall well-being.
Manage complex medication regimens, routinely reviewing and adjusting treatments to minimize risks and adverse effects.
Educate patients, families, and caregivers on preventive healthcare measures, lifestyle modifications, and disease management.
Uphold the highest standards of professional practice in compliance with healthcare regulations and ethical guidelines.
Engage in continuous professional development and quality improvement initiatives to enhance geriatric healthcare services.
Essential Job Duties:
Provide assessment, planning, primary care, and continuous medical coverage to participants
Collaborate with community-based physicians and IDT in discharge planning from acute or long-term settings
Actively participate in IDT, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery
Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
Take part in rotating night and weekend call
Qualifications
Possession of an MD or DO degree from an accredited medical institution with comprehensive training in geriatric care.
Board certification in Internal Medicine, Family Medicine, or Geriatrics, ensuring advanced clinical competence.
Valid state medical license and demonstrated commitment to meeting all regulatory standards.
Proven experience in diagnosing, treating, and managing complex geriatric conditions in a clinical setting.
Proficiency in employing evidence-based practices to deliver high-quality, patient-centered care for elderly populations.
Exceptional communication skills, enabling effective collaboration with multidisciplinary healthcare teams.
A strong commitment to lifelong learning and continual professional development in primary care and geriatric medicine.
