Summary of the Registered Physical Therapists (RPT):
The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.
Qualifications:
Job Requirements of Registered Physical Therapists (RPT):
- Current Connecticut License as a PT (Physical Therapist)
- Current CPR
- At least one (1) experience working in a Skilled Nursing Facility preferred.
Responsibilities:
Essential Duties & Responsibilities of the Registered Physical Therapists (RPT):
- Creates, implements, and executes patient treatment plans.
- Communicates with patients, families, referring physicians, and other members of the team.
- Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
- Develops and enhances clinical and professional skills through knowledge and professional associations.
- Supports and participates in department operations and development.
- Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
- Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
