Social Worker Assessor

Conduct comprehensive assessments: Analyze medical and psychosocial information, conduct home visits, and evaluate client needs to identify appropriate services.
- Develop personalized care plans: Help clients access home and community-based services that empower them to maintain independence in the least restrictive environment.
- Coordinate and connect: Refer clients to resources, consult with professionals, and coordinate services to ensure a seamless support system.
- Champion client autonomy: Advocate for client rights, promote self-determination, and ensure fair access to vital services.
- Document for impact: Maintain accurate records to track progress, inform service decisions, and contribute to the ongoing improvement of the AERS program.
- Holds a valid State of Maryland social work license (LBSW, LMSW, or LCSW, LCSW-C) or a valid State of Maryland RN license.
- Has 2 years of experience in medical-surgical/geriatric and/or pediatric case management.
- Must be able to pass a thorough background investigation.
- Has reliable transportation and a valid driver's license equivalent to a noncommercial, class C Maryland driver's license.
- MUST BE WILLING TO TRAVEL TO CLIENT HOMES THROUGHOUT BALTIMORE CITY MARYLAND.
