Direct Support Professional I
Overview:
Provide direct support to individuals served including providing assistance in personal care activities.
Perform the required training and documentation necessary for group and individual participant goal achievement. Perform dietary and transportation duties.
Responsibilities:
To conduct the follow-up necessary to ensure delivery of Individual Support Plan (ISP) including direct support, reporting concerns to service coordinator and documentation.
Provide assistance as needed for program participants’ personal care needs including dining, toileting, mobility/ambulation and transfers.
To develop and implement positive behavioral supports for individuals as warranted and in conjunction with the Interdisciplinary team.
To develop, maintain and implement individual and group activity programming, with an emphasis on age and diagnosis appropriateness, and the interests, needs and abilities of participants.
To evaluate individuals’ needs and interests by completing participant planning groups as assigned.
To transport participants as assigned including transportation of participants to/from the program in the absence of assigned drivers.Maintain a clean, safe and healthy program environment.
Be familiar with, and assist in the implementation of, the Seven Hills Foundation Policies & Procedures, the site’s Program Specific Policies & Procedures and the regulations governing the program.
To foster the growth of individuals by teaching and offering opportunities for choice, initiation and risk taking. To treat individuals with respect and dignity and in an age and culturally appropriate manner and to respect their privacy and their property.
Qualifications:
High School diploma and one (1) year related developmental disabilities experience or relevant college degree.
Valid Driver’s License required.
