Lead Care Manager

As a Lead Care Manager, you will act as the primary contact person for youth and families requiring care coordination and community linkages. Lead Care Managers play a pivotal role in providing comprehensive care management and support serving as the cornerstone of the care coordination process and the driving force behind ensuring the holistic well-being of high-need youth and families.

The Enhanced Care Management (ECM) program is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need youth and families through the coordination of services, connection to community resources, and comprehensive care management.

Responsibilities:
- Conduct outreach to enroll new clients in ECM, establishing rapport and trust
- Collaborate with youth, families, and care teams to assess needs and develop comprehensive care plans
- Coordinate care activities and maintain communication with external providers including county substance use disorder and specialty mental health providers as appropriate
- Manage referrals, coordination, and follow-up to needed services and supports
- Provide education and identify support needs for youth and their families
- Build and foster collaborative relationships with community resourcing agencies
- Provide services in-person and accompany clients to appointments when needed
- Complete required documentation in a timely manner
- Attend supervision and other team meetings as required
Qualifications:
REQUIRED:
- Additional education/experience requirements:
- Bachelor’s degree in Social Work/Psychology/Counseling + 1 years of experience OR
- Associate’s degree or equivalent + 2 year of applicable experience OR
- High School Diploma/GED + 3 years of applicable experience
- Experience required working with families who are experiencing the impact of complex systematic disparities that contribute to community and individual mental health challenges
- Flexibility to work some earlier mornings or evenings, scheduled ahead of time
- Must be at least 21 years of age
- Own vehicle, Valid California driver's license and insurability with Seneca's automobile policy
- TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
- Must show proof of COVID-19 vaccination
PREFERRED:
- Prior case management and community-based experience
- Bilingual Spanish skill set
- Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks
Schedule:
- Full-time
- Mon-Fri; 9am-5pm
- Flexible schedule with ability to work earlier mornings or evenings as needed
- Combination of in-person & remote work
Benefits:
- Starting range: $25.46 - $27.96 per hour
- Actual salary dependent on creditable experience above the minimum qualification
