Salary: Starting at $47000 / year - depending on experience / education

Job Summary

The Program Supervisor is responsible for the implementation of Individual Support Plans and the holistic care for each resident in their program. The program Supervisor is responsible for assuring adequate staffing to meet program needs in conjunction with the Executive Director.

Additionally, this position is responsible for the supervision, scheduling, evaluation, and discipline of Behavior Support Staff working at their program.

  • Works various scheduled shifts; this is to include at least one (1) weekend shift per month. This position may require a supervisor to work flexible schedules in order to meet coverage needs or to complete job requirements.
  • Program Supervisor will be required to be on a one-week rotational on-call schedule.
  • Develops staffing schedules, assuring that an adequate staffing ratio is always maintained.
  • Reviews and recommends approval/disapproval and forwards requests for vacation and sick accruals to the Executive Director for final approval. Consult HR/Executive Director for any medical leave of absence or personal leave requests.
  • Submits staff vacancies to the Executive Director for final review.
  • Evaluates Behavior Support Staff after a 6-month probationary period and annually on performance evaluation/review forms.
  • Interviews Behavior Support Staff applicants in conjunction with the Executive Director and recommends approval or disapproval of employment.
  • Participates in all team meetings and attends all ISP meetings, day programs and behavioral meetings.
  • Assists with completing all resident assessments and participates in the team process of establishing goals for their assigned residents.
  • Assists the Program Nurse to ensure all medical appointments are completed, current, and followed up when necessary according to DPW regulations.
  • Completes all monthly and quarterly reports on a timely basis for each resident in their program. (Due the 7th of the following month).
  • Approves and submits all maintenance requests to the Executive Director.
  • Responsible for the appropriate expenditure of funds provided for Woods Community Services for the purpose of purchasing items for WCS clients and/or WCS programs. This applies to petty cash and/or client funds and any other purchases for WCS.
  • Responsible for reconciliation of petty cash and client’s funds and completing and submitting monthly ledgers to Accounting and/or Executive Director.
  • Responsible for program vehicle maintenance including ensuring proper registration, inspection, routine maintenance, and repair.
  • Coordinates meetings with all staff members on a monthly basis to review house and resident questions and/or concerns.
  • Attends regular supervisor meetings (usually bi-monthly).
  • Assists with monthly monitoring visits with the resident’s Supports Coordinator
  • Ensures that all program documentation is current, complete, and orderly at the program site, this includes but is not limited to vehicle logs, medical logs, behavior logs, incident reports, daily logs, petty cash logs, monthly fire drills, and Care Logic census and progress notes.
  • Conducts formal program/site orientation and on-the-job training program for Behavior Support Staff.
  • Ensures all staff are meeting their targets and completing their mandatory 24-hour training annually.
  • Initiates/conducts progressive disciplinary policy in conjunction with Executive Director and in consultation with the Human Resources department.
  • Serves as liaison between families and interdisciplinary team on behalf of each resident.
  • Ensures implementation of all professional therapeutic plans.
  • Ensures that all documentation is completed and disbursed to appropriate therapeutic professionals within required timeframes.
  • Communicates daily activities schedule and adjustments to all pertinent staff.
  • Ensures compliance with all 6400 Regulations relating to programmatic operations. (i.e. Individual Records, Medical Records, Fire Safety, etc) and compliance with federal, state, and local licensing and accreditation regulations.

Qualifications:

Qualifications:

  • Bachelor’s degree from an accredited college or university in a related field.
  • A minimum of 3 years work experience with persons with disabilities.
  • At least one (1) year experience working with IDD individuals.
  • Prior supervisory experience preferred
  • Must be able to lift a minimum of 50 pounds.
  • Must have a valid driver's license.
