Impact Recruiting Solutions is currently seeking a Home Health Physical Therapist to fill an opening with a Home Health Aide Services located in San Joaquin County, California.
Requirements of the Home Health Physical Therapist:- Must be a Certified Physical Therapist
- Must have an active state license
- Must have at least 1 year experience in a Home Health or Hospice
- The salary for this position is $110,000 - $120,000 | yr
- Medical, Dental and Vision
- 401K
- Relocation expenses
- This is a Full-time position
- Monday - Friday
- 9:00 AM - 5:00 PM
- Consult with patients to learn about their physical condition.
- Assess and interpret patient evaluations and test results.
- Develop treatment plans using a variety of treatment techniques.
- Create personalized fitness-oriented healthcare programs for patients.
- Administer medically prescribed physical therapy treatments to relieve pain and improve mobility.
- Advise patients on exercise techniques.
- Advise patients and their families about in-home treatment options.
- Provide educational information about injury prevention, ergonomics, and ways to promote physical health.
- Consult and collaborate with other healthcare professionals.
- Document patient care history.
- Comply with rules, regulations, and procedures.
