Job Description:

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care in a Rehab & Long-Term Care setting. The LPN will assist residents with daily activities, administer medications, monitor vital signs, and collaborate with healthcare professionals to ensure optimal patient outcomes.

Responsibilities:

• Assist residents with daily living activities (bathing, dressing, grooming, toileting).
• Administer prescribed medications and monitor for side effects.
• Record and monitor vital signs (blood pressure, temperature, pulse, respiration).
• Provide wound care, repositioning, and mobility support to prevent complications.
• Maintain accurate medical records, treatment plans, and progress notes.
• Communicate with RNs, physicians, and therapists to coordinate care.
• Identify and report any changes in residents’ conditions.
• Ensure compliance with hygiene, infection control, and safety protocols.

Requirements:

• LPN License (Active & in good standing)
• AHA BLS Certification
• 2+ years of recent experience in a Nursing Home or Rehab Center
