Retention Specialist (Part-Time/Temporary)
Overview:
The Retention Specialist supports the Clinical Care Teams to ensure patient care retention by utilizing internal data to determine patients who are at risk of falling out of care. The Retention Specialist will be responsible for contacting patients to remind them of their follow-up appointments, scheduling appointments as needed/required, and communicating any patient requests and/or concerns that are identified during outreach to the Clinical Care Teams and Front Office Staff.
This is a part-time and temporary clinic-based position located in the Midtown area of Atlanta, GA (2140 Peachtree Road).
The work assignment could range from two to three months (concluding no later than June 30, 2025).
The work schedule will be three days a week from 8:00 a.m. - 4:30 p.m.
About the Role:
- Reviewing a roster of patients who are at risk of falling out of care
- Contacting patients in a timely manner, primarily by phone, to schedule appointments; communicates with patients to ensure follow-through with appointments
- Maintains record of patient interactions; tracking patients who are no longer appropriate for clinic follow-up (i.e., moved out of state, deceased, in care elsewhere, etc.)
- Applies a high degree of accuracy in obtaining and verifying patient circumstances, scheduling details, and follow-up to Care Teams
- Responsible for liaising with counterpart Retention Specialist to ensure coordination and minimizing duplication of efforts
- Acts as liaison between AvitaCare Atlanta Care Teams, Front Desk Staff, Clinician Lead, and other key staff members
- Completes all administrative functions related to retention activities in an efficient manner
- All other duties as assigned
About You:
- High school diploma or GED required
- Bachelor's degree preferred in a healthcare-related field
- Bilingual in Spanish is a plus but not required
- Two years experience in a medical patient support service and ambulatory capacity
- Ability to utilize a high level of effective communication skills both orally and written with patients, clinicians, co-workers, administration, and the public.
- Ensures strict confidentiality of all health records, member information and comply with HIPAA requirements
- Display the ability to multi-task in a fast-paced environment
- Usage of general office equipment; MS Office computer skills with the ability to type 30+ wpm
- Embrace and perform all duties in the spirit of the company’s core values and goals
