Responsibilities:
The AID Patient Intake Coordinator in our Outpatient department is primarily responsible for handling external and internal requests for information and services for all levels of care at PrairieCare. Complete Mental Health Screens over the phone with patients seeking services and schedule Admission Diagnostic Assessments and intake appointments for both program and outpatient clinic services as appropriate.Triage crisis phone calls, provide county crisis resources, and complete safety planning. Coordinate internal and external direct referrals to our services.
Responsibilities and Accountabilities:
- Review, assess and link callers with information about our services.
- Re-direct calls to emergency services as needed.
- Provide county crisis resources as needed.
- Triage crisis calls, assess for safety, and complete safety planning as needed.
- Complete Mental Health Screens over the phone.
- Schedule Admission Diagnostic Assessments.
- Schedule intake appointments for both program and outpatient clinic services.
- Facilitate internal referrals for services.
- Facilitate external direct referrals for services.
- Consult with physicians as needed to facilitate direct referrals into our programs.
- Monitor online channels for patient inquiries through our website and follow up with patients as appropriate.
- Provide effective customer relations pursuant to the policies of Prairie Care.
- Provide external referral resources as needed.
- The nature of this position is highly variable. At times individuals in this role may be asked to complete Needs Assessments. While this is not the primary function of this role, staff may be asked to provide support in this manner.
- Located in our Medical Office Building in Brooklyn Park
Hours: Monday - Friday 8:30am- 5:00pm
Qualifications:
- Requires a Bachelor’s degree in Social Work, Sociology, Psychology or a closely related human services field.
- Two years of supervised experience in behavioral healthcare, meeting the criteria for a mental health practitioner or a license as a Licensed Social Worker (LSW) preferred.
- Experience in triage of psychiatric emergencies is preferred.
- Knowledge of DSM-5 criteria, possess interviewing and assessment skills and display a professional attitude in all aspects of his/her performance.
- Oriented and educated in gender, cultural differences and in age appropriate care for psychiatric and chemical dependency services.
- Must possess skills to communicate clearly and effectively with Clinicians.
- Must have computer skills using Microsoft Office Products including Word and Outlook.
- Must have reliable transportation and telephone.
- Ability to smoothly and rapidly shift between dissimilar tasks.
- Ability to communicate well both verbally and in writing.
- Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations.
- Ability to make effective decisions while in stressful situations and circumstances.
- After training, be able to manage disruptive and explosive patients.
