Job Title: Mental Health Case Manager
Location: St. Paul, MN

Duration: 6 months, with potential for extension

Job Summary:
Seeking a Case Manager to provide ongoing support to participants in the Mental Health Court (MHC) program. This role ensures a consistent Case Manager presence at team meetings and court sessions, facilitating comprehensive and coordinated services for clients.

The Case Manager will work closely with the MHC team, including Probation Officers and other case managers, to monitor participant compliance with treatment and court conditions and to support participants through referrals, intake, and supervision.

Responsibilities:

Intake and Participant Engagement
  • Meet with new referrals, complete intakes, and develop personalized treatment plans.
  • Provide ongoing monitoring and updates on participant compliance with court requirements.
  • Ensure that sufficient information is provided to the MHC team to apply incentives or sanctions as needed.
Court and Meeting Attendance
  • Attend MHC staffing sessions and court sessions as scheduled.
  • Participate in bi-monthly Team Alignment Meetings and quarterly Treatment Court Partner Meetings.
  • Assist participants with court forms and applications as needed.
Participant Support and Compliance Supervision
  • Provide comprehensive supervision of participant compliance through direct contact, home visits, and community engagement.
  • Act as the primary liaison between participants, community providers, and probation officers.
  • Support participants in identifying triggers and developing strategies for mental health stability and law-abiding behavior.
Documentation and Program Development
  • Regularly update participant files to reflect progress and compliance.
  • Serve as liaison with other court departments and agencies for cohesive program support.
  • Contribute to MHC program improvements and attend relevant trainings, both in-state and out-of-state.
  • Provide training to replacement case managers if required.
Additional Duties
  • Maintain a regular work schedule, document hours, and report monthly.

Preferred Qualifications:

  • Experience working with diverse populations, justice-involved individuals, and those with mental or chemical health concerns.
  • Previous work within a Mental Health Court environment and familiarity with services in Ramsey County.
  • Licensures such as LICSW, LPCC, LMFT, or LP are a plus but not required.

Schedule:

  • Mental Health Court Staffing: Mondays, 10:00 am - 12:00 pm, 3 weeks per month
  • Mental Health Court: Mondays, 1:30 pm - 4:30 pm, 3 weeks per month
  • Team Alignment Meetings: 1 hour, bi-monthly
  • Treatment Court Partner Meetings: 1 hour, quarterly
