Responsibilities:

The Scheduling Coordinator is responsible for the creating and maintaining clinician schedules according to authorizations, patient needs, and clinician's availability. The Scheduler serves as the point of contact for all interactions with clinical team members, administrative associates, referral sources, and patients.

Partners with clinical leadership to direct the coordination of care.

Essential Job Functions/Responsibilities
  1. Understands and adheres to agency policy and procedures.
  2. Confers with Executive Director on problems and complaints.
  3. Liaises between coordinating team and nursing office.
  4. Performs all coordinating duties when necessary.
  5. Enters scheduling data, creates schedules.
  6. Schedules client appointments/visits according to care plans and staff availability.
  7. Reviews and directs case management notes, documenting change of aide, change in service, telephone complaints, etc.
  8. Contacts care providers, clients, and staff with day-to-day changes.
  9. Maintains staff and client statics reports.
  10. Supervises the scheduling of appropriately skilled person to fill the case, the inputting of the scheduling information on the computer and continue to supervise the coordination of the scheduling.
  11. Supervises the scheduling of replacement workers as needed and the input of scheduling information.
  12. Communicates with patients/families on home health aide related issues.
  13. Coordinates patient care among disciplines.
  14. Performs general office duties including but not limited to word processing, photocopies files, shreds, sort/distributes mail, provides reception and telephone service.
  15. Takes telephone referral information.
  16. Handles matters related to staff and the patients they serve.
  17. Provides staffing for sick leave, vacation, long term leave.
  18. Communicates on a routine basis with contract agencies to maintain smooth relations and to obtain needed information.
  19. Advises the Executive Director of all new cases to be filled and supervises the review of cases on a weekly basis.
  20. Oversees the documentation and reporting of changes in patient status, in-service requirements, in and employee staff assignment and, any unsatisfactory performance.
  21. Discerns client services required as outlined in agreements, urgent requests, and care plans.
  22. Supervises staff regarding punctuality, assignments and reliability.
  23. Performance of other duties as required.
  24. Must adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality, and consistent patient care.

Qualifications:

  1. High School Diploma required.
  2. Minimum of one year of experience in home care agency.
  3. Demonstrates an ability to work with other health care clinicians, development of home plan of care, and knowledge of third party reimbursement.
  4. Self-directed and able to work with minimal supervision.
  5. Demonstrate excellent observation verbal and written communication skills.
  6. Ability to multi-task and work with a team environment.
  7. Personal Car for travel and a valid driver's license.
  8. Maintain personal auto liability insurance coverage.
